Employees all accounts?

this is maybe a direct question to Tom but it is always interesting to here other peoples solutions.

when you are a small / medium businesses you often have tons of different accounts on different websites, mails, logins to ISPs, re-sellers, internal systems etc… how do you have a good control on all the usernames and passwords? even of employees that have quit but where the accounts are still there. do you at LS use the wiki for that to??

We are still using LastPass for managing passwords.
